  • Authorization letter issued by the Kentucky Racing Health & Welfare Fund
  • Kentucky Horse Racing Commission License - Current year
  • $10 cash co-pay
You must have a current KHRC license and present a payroll check or stub showing that you earned at least $150 per week in the most current pay period before an appointment will be arranged. An authorization form will be provided to you that you must bring to the Clinic at the time of your appointment.

Indirect Cost Policy

Due to the nature of funding for the Kentucky Racing Health and Welfare Fund, Inc. and its Health Services Center, and to maximize its resources to go to the benefit of those individuals that it serves, the Kentucky Racing Health and Welfare Fund, Inc. has established the following policy with regard to the payment of indirect costs.

It is the policy of the Kentucky Racing Health & Welfare Fund, Inc. to not allow indirect costs to be charged on any grants, awards, contracts, service agreements, or other similar agreements. Only direct expenses (without indirect or overhead charges) are allowed to be paid to awardees, service providers, contractors, and their subcontractors or providers.
